Adding members who request to join via the "pending request" form is suboptimal

Detailed description
Here are the various problems:

- the mail sent to admins when a user request to be added does not have a valid from that can be used to engage a discussion with the user
- the user can request access without telling why he requests access, he shouldn't be able to let the field empty
- when the admin approves the request, the user doesn't receive a notice that he has been added to the project
- when the admin rejects the request, he doesn't have the possibility to send an explanation to the user why his request he rejected
- when an admin approves/rejects a request, the other admins are not informed by email, they have to fire up the web browser to discover that someone else already handled the request

It would be nice to see this fixed, as some team handle many requests (Perl team, python team, collab-maint, etc.).
